Lake County Student Rides
Parent Transportation Agreement, Terms & Conditions, Liability Waiver, Privacy Notice, and Emergency Authorization
1. Parent/Guardian Certification
By registering for services, signing this agreement, or clicking "I Agree," I certify that I am the parent or legal guardian of the student listed on this account and have the legal authority to enter into this agreement on the student's behalf.
2. Transportation Authorization
I authorize Lake County Student Rides ("Company") to transport my child to and from school, school-related activities, extracurricular programs, tutoring sessions, athletic events, and other destinations approved by me.
I understand that transportation services are provided according to scheduled routes, availability, weather conditions, traffic conditions, and operational requirements.
3. Parent Responsibilities
The parent or legal guardian agrees to:
- Provide accurate student information.
- Provide accurate pickup and drop-off locations.
- Maintain current emergency contact information.
- Notify the Company of any schedule changes.
- Ensure the student is prepared and available at the designated pickup location.
- Inform the Company of any medical conditions, allergies, disabilities, or special accommodations that may affect transportation safety.
The Company shall not be liable for issues arising from inaccurate information provided by parents or guardians.
4. Student Safety Rules
For the safety of all students:
- Students must remain seated while the vehicle is moving.
- Seat belts must be worn when available.
- Students must follow all driver instructions.
- No physical violence, bullying, harassment, or threatening behavior is permitted.
- No destruction of company property is permitted.
- No illegal substances, tobacco products, alcohol, vaping devices, or weapons are permitted.
Violation of these rules may result in suspension or termination of transportation services.
5. Authorized Pick-Up and Drop-Off
Students will only be released to:
- The approved destination listed by the parent or guardian.
- Individuals specifically authorized in writing by the parent or guardian.
Parents are responsible for keeping authorized contact information current.
6. Schedule Delays
The Company cannot guarantee exact pickup or arrival times.
Delays may occur due to:
- Traffic
- Weather
- Vehicle maintenance
- Accidents
- Road closures
- School schedule changes
- Other circumstances beyond the Company's control
The Company shall not be liable for delays caused by circumstances beyond its reasonable control.
7. Emergency Medical Authorization
In the event of a medical emergency involving my child, I authorize Lake County Student Rides and its representatives to:
- Contact emergency services (911)
- Seek emergency medical treatment
- Contact emergency contacts listed on my account
I understand that the Company does not provide medical services and that I am responsible for any medical expenses incurred.
8. Limitation of Liability
To the fullest extent permitted by Florida law, Lake County Student Rides, its owners, employees, drivers, contractors, agents, and representatives shall not be liable for indirect, incidental, special, or consequential damages arising from transportation services.
Nothing in this agreement shall limit liability for gross negligence, willful misconduct, or conduct prohibited by law.
9. Release and Waiver
I voluntarily assume all ordinary risks associated with transportation services.
I agree to release and hold harmless Lake County Student Rides, its owners, employees, drivers, contractors, and representatives from claims, demands, losses, damages, costs, or expenses arising from participation in transportation services, except where caused by gross negligence, reckless conduct, or intentional wrongdoing.
10. Payment Policy
- Payment is due according to the selected transportation plan.
- Late payments may result in suspension of services.
- Returned payments may incur additional fees.
- Services may be terminated for non-payment.
11. Cancellation Policy
Parents may cancel services by providing written notice.
Refunds, if any, will be issued according to the Company's current refund policy.
12. Privacy Policy
Lake County Student Rides respects the privacy of students and families.
Information collected may include:
- Student name
- Parent name
- Address
- School information
- Contact information
- Emergency contacts
- Transportation schedules
Information is collected solely for transportation, safety, communication, billing, and legal compliance purposes.
The Company does not sell personal information to third parties.
13. Photo and Video Policy
Photographs, recordings, or student information will not be publicly used for advertising, promotional materials, or social media without written parental consent.
14. Electronic Communications
By creating an account or registering for services, I consent to receive communications related to transportation services through:
- Text messages
- Telephone calls
- Website notifications
15. Changes to Terms
Lake County Student Rides reserves the right to modify these Terms and Conditions at any time.
Updated terms will be posted on the Company's website.
Continued use of services after updates constitutes acceptance of the revised terms.
16. Governing Law
This agreement shall be governed by the laws of the State of Florida.
Any legal dispute arising from this agreement shall be handled in the appropriate courts located within the State of Florida.
